RiskReady: A Systems Approach to Improving the Recognition and Treatment of High-Risk, HR+, HER2- Early Breast Cancer

Treatment with an adjuvant CDK4/6 inhibitor + endocrine therapy (ET) has been shown to significantly reduce the likelihood of recurrence in patients with high-risk HR+, HER2- early breast cancer (EBC). Now standard of care based on positive outcomes in large clinical trials, this approach has been further validated by expanded regulatory approvals and inclusion in clinical practice guidelines. To ensure optimal implementation of this therapeutic strategy, PeerView and the University of Florida have partnered to develop and deliver a health system–based, multifactorial quality improvement (QI) and educational initiative that features a comprehensive cancer registry, a structured follow-up protocol, and an enduring certified activity. By documenting, tracking, and reporting the clinical, pathological, and biological factors used to identify patients at high risk for recurrence as well as important data on treatment selection and delivery, the registry will provide essential information on risk stratification and treatment of patients with HR+, HER2- EBC seen in the University of Florida health system. Artificial intelligence may be leveraged to aid in patient identification and treatment tracking. Furthermore, to improve treatment monitoring in the critical period shortly after treatment initiation, members of the care team will conduct structured calls with patients to assess for AEs, reinforce education on symtom reporting and AE mitigation and management strategies, and ultimately encourage improved treatment adherence and persistence. An enduring certified educational activity will reinforce learning in clinicians within the University of Florida and extend best practices and tools to those in other institutions.

1. Employ evidence-based strategies to identify patients with HR+, HER2- EBC at high risk of recurrence, recommend appropriate adjuvant therapy, and monitor patients to detect/mitigate AEs and maintain adherence/persistence
2. Identify opportunities for process and workflow improvements within a healthcare system to enhance the identification, treatment, and monitoring of patients with high-risk HR+, HER2- EBC

Credit Designation

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 0.5 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program.
